Job summary

VitalCaring is seeking a compassionate Home Health Aide to provide hands-on care to patients in their homes. You will support daily living activities, monitor health, and document care in the EMR while collaborating with nurses and therapy teams.

Qualifications

• Meets state and federal requirements for Home Health Aide certification or training.
• Current CPR certification is required or on record.
• Valid driver’s license, reliable transportation, and auto insurance.
• Ability to read, follow care plans, and document services provided.
• Ability to work independently with strong time management skills.

Responsibilities

• Assist patients with daily living activities (bathing, dressing, grooming, toileting).
• Support mobility and transfers in line with care plans.
• Maintain a clean, safe, and comfortable home environment.
• Prepare meals and assist with feeding as needed.
• Take and record vital signs when trained to do so.
• Observe and report changes in patient condition to the clinical team.
• Document care provided accurately in the EMR system.
• Participate in team meetings, trainings, and care coordination activities.
• Provide compassionate support to patients and families during each visit.
