Qualifications

• Minimum high school diploma or equivalent.
• State-specific aide certification or licensure required.
• Valid state driver’s license and auto liability insurance.
• One year of Home Health or Hospice Aide or Nursing Assistant experience preferred.

Responsibilities

• Work with the RN to fulfill the patient care plan.
• Collaborate with care team to address all patient needs.
• Provide hands-on personal care and assist with transfers/ambulation.
• Document care delivery daily in the EMR system.
• Contribute to a caring culture through accountability and teamwork.
