Job summary

Always Compassionate Home Care in New York seeks a dedicated Home Health Aide to provide compassionate, professional client care. You will assist with personal hygiene, nutrition, transfers, and daily living activities according to the plan of care and state requirements.

This role values empathy, dependability, and clear communication with clients and families. You will document care, support independence, and collaborate with nursing staff while upholding safety, confidentiality, and quality

Qualifications

• Must be 18 years of age or older.
• Must have a Home Health Aide certificate per State requirements with in-service education to maintain the certificate.
• Maintain good work relationships with client, family and agency personnel.
• Demonstrate genuine care for the sick and disabled.

Responsibilities

• Maintain a healthy, safe environment and provide nutritional and environmental support.
• Assist with personal hygiene, toileting, dressing, walking and feeding the client.
• Position the client as instructed to ensure comfort and safety.
• Assist with therapies and treatments as ordered by physician and nursing supervisor.
• Remind the client to take medications.
• Document on the Aide Charting Sheet daily and report changes to the agency or nursing supervisor.
• Use medical supplies and equipment appropriately.
• Provide emotional support and build trust with the client.
• Encourage client independence.
