Always Compassionate Home Care is seeking a dedicated Home Health Aide to support clients in the New York area. You will follow established care plans, assist with activities of daily living, mobility, and feeding, and provide emotional support to clients in their homes.

Responsibilities include documenting care, reporting changes to the nursing supervisor, and ensuring patient safety. A HHA certificate is required, with ongoing in-service education to maintain credentials.

Qualificações

• Must have a Home Health Aide certificate per state requirements.
• Must demonstrate ongoing in-service education to maintain certification.
• Ability to establish and maintain professional relationships with clients, families and agency staff.

Responsabilidades

• Follow the care plan as outlined by the Nursing Supervisor.
• Assist with personal hygiene, toileting, dressing, walking and feeding.
• Position the client when instructed.
• Assist with treatments and therapies ordered by the physician.
• Remind client to take medications and assist with reminders.
• Provide emotional support and encourage client independence.
• Document daily care on the Aide Charting Sheet.
• Report changes in client condition promptly.
