Job summary

Pathways Healthcare, a leading home health and hospice organization, is seeking a Home Health Aide for Nashua, Merrimack, Londonderry and surrounding areas. The role offers a flexible Monday–Friday schedule with part-time hours and a $25.00/hour rate.

Qualified CNAs in NH are preferred, with HHA certification accepted. CPR is required, and a valid driver’s license with insured vehicle is needed for community visits. Mileage reimbursement and PTO are offered.

Qualifications

• Licensed Nursing Assistant (CNA) in NH preferred; Home Health Aide certification accepted.
• Must be 18 years of age or older.
• Ability to read and follow written care plans and document services provided.
• Ability to work independently with minimal supervision.
• Compassion and empathy for individuals who are ill, injured, or elderly.
• Current CPR certification required.
• Strong interpersonal skills, patience, and professionalism.
• Valid driver’s license and reliable insured vehicle.
• Ability to assist patients with repositioning, transferring, and other physical tasks.

Responsibilities

• Provide personal care assistance such as bathing, grooming, dressing, and mobility support.
• Observe and report changes in patients’ condition to the supervising nurse or therapist.
• Document care provided and patient observations accurately; follow the care plan established by the Registered Nurse.
• Maintain a safe and supportive environment for patients in their homes.
• Promote quality, continuity, and safety of patient care.
