Qualifications

Responsibilities

• Provide personal care and assistance with ADLs (bathing, dressing, grooming, toileting).
• Assist with meal preparation, feeding, and light housekeeping.
• Monitor and report changes in patient condition to the healthcare team.
• Assist with medications and treatments as directed by the plan of care.
• Maintain a safe, clean environment and provide emotional support.
• Collaborate with healthcare professionals to ensure seamless care delivery.
• Document patient information in the electronic health record system.
