Job summary

A Place At Home Towson is seeking a Home Health Aide to provide one-on-one care in patients' homes, following individualized care plans with compassion and attention to detail.

You will document care activities, maintain patient comfort, and communicate clearly with supervisors. Prior HHA or Hospice experience is a plus, with a focus on building meaningful relationships and promoting a healthy environment.

Qualifications

• Must have completed an accredited Home Health Aide program.
• High School Diploma or GED required.
• Driver's license required; CNA/CMT and CPR certifications preferred.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
• Give medications and immunizations
• Engage patients in exercises or other activities
