Job summary

Qualifications

• Experience in home health care or related field preferred but not required.
• Strong observational skills to monitor patient conditions.
• Ability to perform meal preparation, laundry and light cleaning duties.
• Knowledge of hospice care principles a plus.
• CPR certification desirable.
• Compassionate demeanor with excellent communication skills.
• Must be reliable, punctual and able to work independently while following care plans.

Responsibilities

• Assist clients with ADLs: bathing, dressing, grooming, mobility.
• Prepare nutritious meals tailored to dietary needs.
• Monitor health status and report changes to health-care providers.
• Offer companionship and engage clients in meaningful activities.
• Maintain client living environment with light housekeeping.
• Provide memory care support for cognitive impairments.
• Administer CPR if necessary and assist with catheter care as required by the health plan.
