Job summary

PurposeCare is seeking compassionate caregivers to provide one-on-one home health and home care support. You will assist with daily living activities, monitoring client wellbeing, and ensuring safety in the home environment.

Qualifications

• Active HHA or CNA certification (or willingness to obtain)
• Ability to follow written care instructions and document services
• Ability to lift up to 25 pounds
• Reliable transportation and ability to travel to client homes
• Willingness to complete TB testing, background screening, and onboarding requirements
• 6+ months of caregiving or direct patient care experience (nice-to-have)
• CPR and Adult First Aid certification (nice-to-have)

Responsibilities

• Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting
• Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan
• Prepare meals and assist with feeding as needed
• Support medication self-administration through reminders
• Monitor and report patient condition and changes to the supervising nurse
• Take and record basic vital signs when trained and assigned
• Maintain a clean, safe, and orderly client environment
• Travel reliably to patient homes and assigned visits across your service area
• Complete required in-service training, including annual dementia education
