Qualifications

• Must meet state and federal training requirements.
• CPR certification current per state regulations.
• Complete Maxim competency program.
• Pass Maxim screenings.

Responsibilities

• Assist with daily living activities per plan of care.
• Monitor vitals and report changes to nurse.
• Perform safe transfers and ambulation as directed.
• Provide personal care including bathing, grooming, and feeding.
• Document care provided and observations.
• Ensure safety and home safety practices.
• Communicate with client, family, and office.
