Job summary

Heart of Hospice, part of the Optum family, is seeking a Home and Community Based Services Aide to support clients at home in Louisiana. The role focuses on personal care, daily living activities, and coordinating with families to maintain independence in the client’s home.

Requirements include CNA certification (or completion) and a valid First Aid certificate, with CPR preferred. Excellent organization, reliability, and compassionate care are essential.

Qualifications

• Current CNA certification or successful completion of a CNA training program.
• First Aid Certification preferred or obtainable.

Responsibilities

• Provide personal care and assist with daily living activities (bathing, grooming, dressing, ambulation, medication reminders).
• Support household tasks, meal preparation, and accompany clients to appointments or errands.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality using EVV system.
• Ensure a safe environment and operate medical equipment as needed.
• Respond promptly to client needs and adapt to diverse home situations.
