Job summary

Community Hospice is seeking a State Registered Nursing Assistant to provide personal care in patients' homes under the direction of a registered nurse. You will make home visits, perform basic nursing tasks, monitor vital signs and document care in a timely manner.

The role requires a high school diploma, state HHA certification, and at least one year of patient care experience. Expect 40 hours weekly with 8-hour shifts, Monday–Friday, in-person work location.

Qualifications

• High School diploma or equivalent and HHA certification.
• Current health requirements as requested by Community Hospice.
• Completion of state-approved CNA course and test.
• One year of patient care experience is preferred.
• Continuing education required for licensure per KY/Ohio rules.
• Ability to work independently with good communication skills.
• Must pass driving records, background check, and drug screen.

Responsibilities

• Provide personal hygienic care to patients in the home setting under RN guidance.
• Perform routine nursing procedures under direction of a registered nurse.
• Monitor and document vital signs and care activities.
• Discuss visit frequency needs with RN and supervisor.
• Maintain supplies and assist with housekeeping as needed.
• Attend monthly staff meetings and in-service programs.
• Demonstrate proper body mechanics and decubitus prevention.
• Adhere to universal precautions and patient privacy rules.
• Participate in on-call duties as necessary.
• Record care in a legible, timely manner.
