Job summary

Qualifications

• Formal training in Direct Care or related fields required or completed via PACE SEMI program.

Responsibilities

• Provide direct home care in participants' homes or community settings.
• Assist with activities of daily living and essential tasks.
• Support transportation, appointments, and day health center activities.
• Maintain accurate documentation of care provided and visit details.
• Respond to emergencies per policy and report deviations to supervisor.
