Job summary

UnitedHealth Group in Fond Du Lac, Wisconsin is seeking a Per Diem Personal Care Assistant to support clients in their own homes. You will assist with daily living activities, personal care, and light household tasks to help clients remain independent.

This role requires a valid driver’s license with reliable transportation, CPR certification preferred (within 90 days), and 6+ months of home care experience. Flexible hours are available, and benefits are offered where eligible.

Qualifications

• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.
• 6+ months of home care experience.
• Ability to work flexible hours as required to meet identified clients’ needs.

Responsibilities

• Provide assistance with activities of daily living, personal hygiene, and meal preparation.
• Transfer and reposition client safely and support mobility aids.
• Accompany to medical appointments and errands as needed.
• Monitor and document vital signs when ordered.
• Maintain a clean, safe home environment for the client.
