Job summary

Amivie, LLC is seeking a Personal Care Attendant to provide ADL/IADL support in clients’ homes under RN supervision. You will follow the care plan, document time and activities, and ensure safety and compliance with agency policies.

Responsibilities include assisting with bathing, dressing, mobility, meal prep, transportation, and companionship, while adhering to infection control and safety requirements. Previous HCBS experience is preferred and reliable transportation is required.

Qualifications

• Eighteen (18) years of age or older.
• An individual’s criminal history check complying with 6.0 Clinical Coverage Policy 3L.
• Do not have a finding on the North Carolina Health Care Registry in accordance with G.S.131E256
• Experience in Home and Community Based Services, preferred.
• Current Tb Skin Test
• Punctuality
• Desire to help those at home
• Professional demeanor and appearance.
• Have reliable transportation and proof of current automobile insurance.
• Can demonstrate competence providing assigned tasks

Responsibilities

• Comply with all policies and procedures set out by Quality Home Staffing / Community Based Home Care Employee Manual.
• Provide services as assigned by the RN Field Supervisor in accordance with the individual’s approved service plan.
• Report deviations in an individual’s functional or medical condition to the appropriate agency personnel.
• Provide services during the assigned schedule for each individual.
• Report incidents, accidents or other events not aligned with the plan of care.
• Demonstrate competence with assigned ADLs/IADLs.
• Be present for Supervisory Visits by the RN Field Supervisor.
• Accurately report time and attendance by branch office.
• Log time and attendance via EVV or timesheets as required by payer.
• Report concerns of Abuse, Neglect or Exploitation to the office.
• Report planned absences to reassign shifts.
• Advocate for individuals needing other community services or higher care.
• Dress professionally and wear nametag at all times.
• Wear PPE as required during assignment.
• Comply with rules for specific programs.
• Other duties, as assigned.
