Qualifications

• Experience providing home care support and assisting clients with daily living activities.
• Ability to follow a care plan and communicate changes to the care team.
• Strong reliability and punctuality with documentation and shifts.

Responsibilities

• Follow the client’s care plan as outlined by the Care Designer.
• Assist clients with activities of daily living (ADLs) within the scope of practice.
• Support personal hygiene, meal planning and light housekeeping.
• Take notes on changes in client condition and report to supervisor.
• Record hours worked and ensure accurate documentation.
