The aide performs hands-on care, assists with activities of daily living, and documents visits accurately. Training and ongoing education are provided, with opportunities for growth within the team.

Qualifications

• Ability to read and write and follow directions.
• Effective communication to convey care with dignity and respect.
• Mature, reliable and able to handle job demands.

Responsibilities

• Provides services ordered in the plan of care and permitted under state law.
• Performs hands-on personal care.
• Performs simple procedures as an extension of nursing services as assigned.
• Assists with medications that are ordinarily self-administered as assigned.
• Completes visit records in a timely manner per agency policy.
• Reports changes in the patient's condition to the RN.
• Attends meetings and care conferences as scheduled.
• Follows agency policy and maintains patient confidentiality.
