Job summary

Extended Family Home Care Services, LLC is seeking PRN CNAs and Home Health Aides to provide patient services across Southwest Florida. W-2 employer with lots of open hours and a flexible schedule.

Pay range is $20.00–$21.00/hr based on experience and certifications; benefits include 401(k) eligibility after 30 days, paid orientation, and ongoing training. DUTIES include assisting with daily living, following the nurse plan, and maintaining a safe home environment.

Qualifications

• High school diploma or equivalent.
• Certified Nursing Assistant or Certified Home Health Aide.
• Active AHCA Fingerprints (Level II) with Eligible status.
• CPR/First Aid Card from recognized provider.
• TB test within 12 months or Chest X-Ray within 5 years.
• Florida Driver's License with insured vehicle.
• Ability to drive to client homes and access varied homes.
• English communication ability.

Responsibilities

• Provides personal care and related services in the home under supervision of the Supervising Nurse.
• Follows the plan of care to provide safe, competent care for the client.
• Helps with hygiene and maintains a healthy, safe home environment.
• Plans and prepares nutritious meals and assists with ambulation as needed.
• Reports changes to the supervisor promptly and documents visits weekly.
• Coordinates with other staff and community agencies involved in the client's care.
