Job summary

Extendicare is seeking a part-time Health Care Aide to provide personal care and assist with daily living activities for residents. You will follow the plan of care, support mobility and transfers, observe changes, and report to the Client Service Supervisor.

The role requires compassion, excellent communication, and reliability in a long-term care setting. You will help with meals, comfort, and safety, and collaborate with multidisciplinary teams while maintaining confidentiality and adhering

Qualifications

• Graduate of Health Care Aide program (or provincial equivalent certificate with minimum 500 hours in Ontario & Alberta)
• Experience working with seniors in community health services
• Strong written and oral communication skills
• Demonstrated organizational and time management skills
• Current Basic Cardiac Life Support and First Aid certification recommended
• Ability to perform lifting/transferring tasks
• Vulnerable Sector Check within six months or willingness to obtain one

Responsibilities

• Adhere to the plan of care as determined by the supervisor
• Facilitate residents' move in and transfers where applicable
• Observe and report on residents' care using Extendicare forms
• Assist residents with personal care, hygiene, and activities of daily living
• Observe changes in residents' condition and report to supervisor
• Elevate medical emergencies to supervisor or EMS as needed
• Plan and prepare nutritious meals as required
• Assist with ambulation, mobilization, transfers and adaptive aids
• Provide cognitive, social, emotional, and psychological support to residents and families
• Maintain resident information confidentiality
• Address resident/family concerns and escalate as appropriate
• Maintain a safe and clean environment and handle equipment and belongings
• Engage with other agencies involved in resident care
• Attend training sessions to develop knowledge and skills
• Uphold Extendicare's values and Code of Conduct
• Complete other duties as assigned
