Job summary

OneWell Health Care seeks a Home & Community Support Professional to provide direct care and person-centered support for individuals with IDD in home and community settings in Franklin, IN. The role emphasizes independence, daily living skills, safety, and community participation under PCISP guidelines.

The HCSP requires CPR, TB test, background checks, and BDS training completion. Flexible schedule and non-exempt status support field-based work and community integration.

Qualifications

• High school diploma or equivalent required.
• CPR certification required.
• Negative TB test required.
• Criminal Background Check required.
• Federal and State Exclusion Checks required.

Responsibilities

• Provide direct care and person-centered support to individuals with IDD in home and community-based settings.
• Assist with personal care, meal preparation, mobility, daily living skill development, and community participation.
• Implement individualized goals outlined in the PCISP and monitor health, behavior, and safety.
• Document health, behavioral, and safety concerns and maintain accurate daily notes; collaborate with families, guardians, and interdisciplinary team members to ensure quality care.
• Ensure compliance with Indiana BDS requirements, company policies, and documentation timelines.
