Job summary

OneWell Health Care in Indiana is seeking a Home & Community Support Professional (HCSP) to provide direct care and person-centered support to individuals with IDD in home and community settings. You will help with daily living skills, personal care, and community integration, while documenting activities and outcomes in PCISP.

This role requires CPR certification, TB test, background checks, and completion of BDS HCSP training prior to service.

Qualifications

• Requires high school diploma or equivalent.
• CPR Certification required.
• Completion of BDS HCSP training modules prior to providing services.
• Negative TB test required.
• Criminal Background Check required.
• Federal and State Exclusion Checks required.

Responsibilities

• Provide direct care and person-centered support to individuals with IDD in home and community-based settings.
• Assist with personal care, meal preparation, mobility, and daily living skills.
• Monitor health, behavior, and safety; document observations and incidents.
• Collaborate with families, guardians, supervisors, and interdisciplinary teams to ensure quality care and compliance with policies.
