Halcyon Home is seeking a dedicated and compassionate Home Health Physical Therapist (Full Time) to support our geriatric home health population in San Marcos, TX and surrounding areas.

Qualifications

• Current PT license is required.
• Experience with home health or similar setting preferred.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal abilities.

Responsibilities

• Performs ongoing clinical assessment including OASIS assessments to identify patient needs and functioning.
• Develops and evaluates the plan of care with patient and family.
• Maintains documentation and timely clinical notes.
• Applies infection control and standard precautions in patient care.
