Job summary

Fallon Health Weinberg is seeking a dedicated PACE Center Aide to support elderly participants at the Buffalo center. The role involves assisting with activity participation, meal times, and personal care under RN supervision.

Required is a high school diploma or GED, with preference for 1 year of geriatric care experience. Immunizations must be up-to-date, and the position adheres to CMS standards and program rules. Competitive hourly pay is offered.

Qualifications

High school diploma or GED required.
Minimum of one year experience with frail/elderly population preferred.
Empathetic, patient and discreet with good judgment.

Responsibilities

Assists participants with participation in activity programs at the PACE Center.
Sets up PACE center for snacks and lunches.
Set-up and clean-up program areas daily.
Ensure participants can participate in group activities and interact with participants individually.
Communicate problems or needs to the Center Manager.
Assist with progress reports and routine assessments as requested.
Abide by PACE program rules and safety regulations.
Provide meal and snack assistance including feeding as needed.
Provide personal care (bathing, toileting, incontinence care, transfers, lifting).
Assist with laundering participants’ clothes and linens.
