Foundation Partners Group is seeking a Transfer Care Specialist to serve families when arranging transport of the decedent from hospital, nursing facility, or residence to the funeral home. You will remain calm, compassionate, and professional in challenging circumstances.

Key duties include answering questions, directing families to resources, ensuring safe transport, and completing paperwork accurately while adhering to laws and regulations.

Qualifications

• Must have valid state-issued driver's license and a clean driving record.
• Must be able to lift 100 lbs safely.
• Proficient in Microsoft Office Suite (Word, Excel, PowerPoint).
• Demonstrates composure and compassion.
• Ability to follow detailed instructions and apply feedback.
• Ability to establish and maintain effective internal and external work processes.

Responsabilités

• Answer questions and direct families to resources upon arrival and departure for transfer of the decedent.
• Safely transport decedents and navigate local roads and weather conditions.
• Be available for irregular hours including nights and weekends; fill in for staff as needed.
• Ensure paperwork is completed correctly and timely per relevant laws and regulations.
• Receive instructions, determine task order, and complete tasks promptly.
• Maintain a professional appearance and perform other duties as needed.
