Job summary

Senior Care Partners PACE in Albion, MI is seeking caregivers to provide in-home care for participants, including help with shopping, laundry, meal prep, and errands, within their home, day center, or community setting. You will work under guidance from the care team to support participant plans of care.

The role emphasizes helping participants maintain independence, dignity, and engagement, while coordinating with staff and families.

Qualifications

• Must possess, as a minimum, a high school diploma, or equivalent.
• Well-developed ability to show discretion and maintain confidentiality.
• Good communication skills, both written and verbal.
• Detail oriented with good problem-solving skills.
• Basic computer skills.
• Minimum of one year of experience working with the frail, elderly, or long-term care population.

Responsibilities

• Provides ADLs (shopping, laundry, meal preparation, housekeeping, medication reminders, etc.) in participant’s homes that are essential for their health and independence.
• Will observe, record in medical record and report changes in participant’s emotional, behavioral and physical condition to appropriate disciplines and/or leader.
• Encourages each participant’s autonomy and dignity by communicating in a respectful and sensitive manner according to their needs.
• Promotes participants to engage at maximum level of independence during care.
• Must care, support and collaborate with everyone involved in the participant’s care including staff and the participant’s family using the participant centered care model.
