Job summary

Steinbacher, Goodall & Yurchak is seeking an elder care navigator to guide families through care planning within our legal team. You’ll advocate for clients, coordinate with care facilities, and ensure the client’s voice and dignity are central throughout the journey.

You’ll collaborate with attorneys, long-term care planners, and staff to connect families with options, assist with transitions, and maintain confidential information in accordance with firm policies.

Qualifications

• Bachelor’s degree in Social Work or related field (or equivalent work experience).
• 2–3 years of experience in a professional environment with strong administrative, coordination, or client-facing responsibilities.
• Strong organizational skills and ability to manage multiple priorities with consistent follow-through.
• Exceptional verbal and written communication skills.
• High professionalism, integrity, and commitment to confidentiality.
• Proficiency with Microsoft Office Suite and standard office technology.

Responsibilities

• Serve as advocate and elder care professional on the legal team to facilitate care planning.
• Act as liaison between the law firm, care facilities, and clients to facilitate options.
• Provide care coordination to guide clients through facility placement or transfer.
• Attend Interdisciplinary Team Meetings at nursing facilities and document notes.
• Assist with life care planning and research community resources.
• Represent the firm to facilities and healthcare community through visits.
• Collaborate with marketing staff to promote services across the service area.
• Maintain client confidentiality per firm policies.
