Qualifications

• Ability to communicate (verbal and written) with all levels of personnel.
• Meet state-specific licensure and program requirements.

Responsibilities

• Perform personal care tasks including hygiene, grooming, feeding, and ambulation.
• Provide emotional support and monitor health to promote independence.
• Implement Individual Service Plan and Behavior Support Plans.
• Document services provided and maintain accurate records.
• Transport individuals to community activities and appointments.
• Assist with meal planning, preparation, and housekeeping tasks.
• Ensure client safety and a safe home-like environment.
