Qualifications

• High school diploma or GED required.
• Entry-level; on-the-job training provided.
• Reliability and ability to work in home/community settings.
• Familiarity with EHR systems or willingness to learn.

Responsibilities

• Provide care in the home of the individual and the community they live in.
• Assist with daily living tasks such as housekeeping, meals, bathing, dressing, and employment support.
• Transport the client to appointments and run errands as needed.
• Help plan and organize their schedule.
• Daily documentation and reporting in the electronic health system of record.
