Zusammenfassung

Alpine Home Health MN seeks a compassionate Direct Support Professional (DSP) to assist individuals with disabilities in accordance with 245D regulations. The DSP will provide direct care, follow individualized support plans, and document services in a timely manner.

Responsibilities include accompanying clients to appointments and activities, driving when needed, and using the Electronic Verification Visit App to log hours.

Qualifikationen

• Valid driver's license required.
• Reliable transportation preferred (not required for night supervision).
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Must pass MN Dept of Human Services background check.
• Not excluded from Federal or State Healthcare Programs.
• Complete Orientation and Positive Support Rule Training (online or blended).
• Must complete Vulnerable Adult Training through DHS.
• Must be able to stand, walk, bend and stoop to care for individuals served.
• English communication required to perform duties.

• Provide direct care and support to individuals with disabilities in a respectful and compassionate manner.
• Implement individualized support plans as established by the care team.
• Accompany and drive individuals to appointments, community activities, and run errands.
• Maintain accurate and timely documentation of services provided.
• Log hours and services using the Electronic Verification Visit App.
