Job summary

alpinehomehealthmn is seeking a compassionate Direct Support Professional (DSP) to support individuals with disabilities under 245D regulations. You will provide direct care, implement care plans, accompany clients to appointments, and document services with accuracy.

The role requires flexibility to work mornings, evenings, weekends, and holidays, plus the ability to pass safety checks and training. This position offers growth opportunities and competitive pay.

Qualifications

• Valid driver's license and reliable transportation (not required for night supervision).
• Ability to work flexible hours, including weekends and holidays.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Pass Minnesota DHS background check.
• Not excluded from Federal or State Healthcare Programs.
• Complete Orientation and Positive Support Rule Training (online or mixed).
• Required Vulnerable Adult Training.
• Physical ability to stand, walk, bend, and stoop to care for individuals.
• Must be able to move up to 40 pounds.

Responsibilities

• Provide direct care and support to individuals with disabilities in a respectful and compassionate manner.
• Implement individualized care plans and behavior support plans as established by the care team.
• Accompany and drive individuals to appointments, community activities, and run errands.
• Maintain accurate and timely documentation of services provided.
• Utilize the Electronic Verification Visit App to log hours and services.
