alpinehomehealthmn is seeking a compassionate Direct Support Professional (DSP) in Minnesota to provide direct care to individuals with disabilities in accordance with 245D regulations.

You will implement individualized support plans, help clients attend appointments and community activities, and document services accurately. The role requires flexibility for weekends/holidays and the ability to work independently or as part of a team.

Qualifikationen

• Must have valid driver's license and reliable transportation.
• Ability to work flexible hours, including weekends and holidays.
• Excellent verbal and written communication.
• Ability to work independently and as part of a team.
• Must pass MN DHS background check and Vulnerable Adult training.
• Ability to use Electronic Verification Visit App to log hours.

Aufgaben

• Provide direct care and support to individuals with disabilities with compassion.
• Implement individualized support plans by the care team.
• Accompany and drive individuals to appointments and activities.
• Maintain accurate documentation of services provided.
• Log hours and services using the Electronic Verification Visit App.
