Alpine Home Health MN is seeking a compassionate Direct Support Professional (DSP) to support individuals with disabilities under 245D regulations. You will provide direct care, implement support plans, and accompany clients to appointments and activities.

Ideal candidates have strong communication and teamwork skills, can work weekends, and can complete required training and background checks. A reliable vehicle and valid license are needed, with flexible hours including holidays.

Qualifikationen

• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Pass MN Dept of Human Services background check.
• Not excluded from Federal or State Healthcare Programs.
• Ability to complete Orientation and Positive Support Rule Training online or in person.
• Required Vulnerable Adult Training through the Department of Human Services.
• Must be able to stand, walk, bend and stoop to provide care.
• English must be sufficient to understand instructions and document care.

Aufgaben

• Provide direct care and support to individuals with disabilities in a respectful manner.
• Implement individualized support plans as established by the care team.
• Accompany and drive individuals to appointments, activities, and errands.
• Maintain accurate and timely documentation of services provided.
• Log hours and services using the Electronic Verification Visit App.
