Qualifications

• Proof of High School Diploma/GED.
• Valid driver’s license to ensure ability to drive agency vehicles with clean MVR.
• Medication certification within provisional period.
• Ability to write legibly.
• Ability to communicate effectively with other disciplines and individuals served.

Responsibilities

• Assists individuals with daily living activities and life skills.
• Provides direct care per agency policy.
• Ensures safety and therapeutic environment.
• Dispenses medication per agency policy.
• Transports individuals as required by supervision level.
• Participates in treatment team or recommendations.
