Qualifications

• High school diploma or GED required.
• Ability to read, write and comprehend English to follow care plans.
• Experience in training, case management and service delivery to adults with ID/DD.
• MAP Certification training offered within 6 months of employment; CPR/First Aid offered within 3 months.

Responsibilities

• Assist individuals with daily living activities including hygiene, bathing, dressing, feeding and meal prep.
• Transport individuals using wheelchair-accessible vans and assist during appointments and community activities.
• Complete ISP and daily logs, track behavioral data to promote independence.
• Provide companionship, guidance, and redirection while maintaining safety and dignity.
