Qualifications

• Vital signs monitoring per schedule and policies.
• Medication administration per physician orders.
• Assisting residents with mobility and daily living activities.
• Maintaining accurate documentation of resident information.
• Using gait belts and safety procedures during transfers.
• Completing Relias assignments as required by the community.

Responsibilities

• Assist residents in and out of bed and into wheelchair as needed.
• Assist with bathing, dressing, and peri care.
• Assist to dining area and assist with feeding as needed.
• Take and record vital signs per schedule.
• Maintain med room in a clean and orderly state.
• Pass medications and perform treatments as ordered by physician.
• Provide housekeeping and laundry services as scheduled.
• Document and report resident information before shift end.
