Job summary

InnovAge operates Adult Day Health Centers that help seniors live independently within their communities. The Day Center CNA assists participants with activities of daily living, follows care plans, and provides restorative and supportive care.

You will document care, monitor health status, and collaborate with RNs and the interdisciplinary team, including helping with meals, transfers, and transportation support as needed.

Qualifications

• Active and valid state-issued CNA license.
• Current CPR certification or the ability to obtain CPR certification.
• High School Diploma or GED equivalent.

Responsibilities

• Assist participants with daily activities and ADLs as per care plan.
• Document care provided and report to RN per policy.
• Monitor health status and vital signs; report changes promptly.
• Provide restorative and supportive care and assist with meals and transfers.
• Assist with transportation-related duties and group activities as needed.
