Job summary

InnovAge is seeking a Day Center CNA to support adults at our Adult Day Health Center, assisting with ADLs and providing restorative care. You will follow care plans, document care, and collaborate with the RN and multidisciplinary teams to plan patient activities.

Qualifications include an active CNA license and CPR certification, with a High School diploma or GED. This on-site role offers comprehensive benefits and a pay range of $18.37–$21.15 per hour.

Qualifications

• CNA license current and in good standing.
• CPR certification or ability to obtain.
• High School Diploma or GED required.

Responsibilities

• Assist participants with ADLs and daily activities.
• Document care per policies and regulations.
• Monitor health status and report to supervisor.
• Provide restorative and supportive care and assist with transfers.
• Transport support and loading/unloading from vehicles.
• Participate in group and individual activities as scheduled.
