InnovAge Holding Corp. seeks a Day Center CNA to support participants at our Adult Day Health Center. You will assist with ADLs, provide restorative and supportive care, monitor health status, and collaborate with a multidisciplinary team to ensure personalized care plans.

Responsibilities include documentation, activity facilitation, and participation in ongoing training. This full-time role offers a comprehensive benefits package and a competitive hourly wage.

Qualifications

• Active and valid state-issued CNA license.
• Current CPR certification or ability to obtain CPR certification (AHA/ARC accepted).
• High School Diploma or GED equivalent.

Responsabilités

• Assist participants with ADLs per care plan (bathing, dressing, grooming, hair care, mouth care, eating, toileting).
• Document care provided following policies and regulations.
• Record and report care in a timely, effective manner under RN supervision.
• Provide restorative and supportive care including ambulation, positioning, transfers, skin care, and other assigned tasks.
• Monitor health status (vital signs, weight, skin, appetite) and report changes to clinical supervisor.
• Collaborate with multidisciplinary care planning teams.
• Assist with meals and feeding as needed.
• Load/unload participants from vehicles and support transportation needs.
