Job summary

Pinnacle Home Care in Spring Hill, FL, seeks a full-time Certified Occupational Therapist Assistant (COTA) to join our award-winning home health team. Under the OT's supervision, you will provide client care, assist in evaluating and developing the rehabilitative plan of care, and aid in discharge planning.

Qualifications

• Graduate of an accredited occupational therapy assistant program and license/certification as required by state law.
• Current BLS certification required.
• 6 months to 1 year of home health experience preferred; strong communication and organization skills.

Responsibilities

• Under OT direction, provide delegated client care and assist in evaluation and plan of care.
• Prepare and review clinical notes and periodic summary reports.
• Instruct clients and families in exercises and self-care activities.
• Document outcomes and coordinate with OT and physicians on visits and discharge.
• Assist in scheduling, reassessment, and coordination with the clinical team.
