Job summary

Centennial Post Acute is seeking a Certified Occupational Therapy Assistant (COTA) to join our compassionate team. You will work with residents to restore independence, document care, and collaborate with an interdisciplinary team to support resident progress.

Responsibilities

• Collaborate with residents, families, physicians, and team members to ensure the best care.
• Provide timely, accurate documentation meeting facility, insurance, and regulatory requirements.
• Enhance clinical skills through training and educational opportunities.
• Report changes in resident status to supervising therapist to adjust treatment plans.
• Ensure safe, efficient environments for residents and staff, contributing to overall well-being of the facility.
• Coordinate treatment goals and provide feedback to rehab aides to ensure high-quality care.
