Job summary

Easterseals PORT Health is seeking a full-time Community Support Team Qualified Professional to serve in Surry and Yadkin Counties, NC. You will deliver community-based services to adults with mental health or substance use disorders, applying structured rehabilitative techniques in home and community settings.

We offer a competitive hourly wage between $21.00-$24.00, a Monday–Friday schedule, and comprehensive benefits.

Qualifications

• Bachelor’s degree from an approved accredited institution.
• Minimum of two years’ experience working with the Adult Mental Health population.
• Valid driver’s license, current auto insurance and a good driving record.
• Demonstrate high moral and ethical character, and be free of any substantiated charges of abuse or neglect on the NC Healthcare Registry.

Responsibilities

• Provide services to individuals who have a mental health or substance use disorder and meet the two life domains.
• Participate in the provision of structured rehabilitative interventions to individuals, such as housing and tenancy, money management, daily activities and personal wellness.
