• Airborne and bloodborne pathogen training with annual updates.

Responsibilities

• Document all tasks in a timely, complete, and accurate manner.
• Collaborate with patient, physician, and health care team.
• Follow the Plan of Care/Home Health Aide Assignment sheet for each shift.
• Alert RN Case Manager to significant changes in patient status.
• Report care results to the RN in a timely manner.
• Adhere to emergency action plans and agency policies.
