Job summary

Split Rock Rehabilitation and Health Care Center in New York is seeking a compassionate CNA to assist licensed nursing staff with bedside and out-of-bed care, following facility policies, and maintaining resident dignity and privacy.

You will participate in shift reports, rounds, and care planning; document care and observations; escort residents to appointments; and help with meals, hygiene, mobility, and daily tasks.

Responsibilities

• Attends beginning of shift report to be aware of residents' condition.
• Makes preliminary rounds to assess assignments.
• Confers with the Charge Nurse for clarity of assignment as needed.
• Keeps confidential all knowledge of residents' problems/conditions.
• Administers care in accordance with policies.
• Escorts residents to clinics/outside physicians when assigned.
• Completes accountability records per facility policy.
• Offers suggestions to upgrade quality of care and participates in care planning.
• Reports pertinent information (skin issues, temp, complaints, incidents, changes in patient condition).
• Notifies if resident refuses fluids or has poor appetite or refuses treatment.
• Notifies if problems require Housekeeping or Maintenance services.
• Provides personal hygiene tasks (showers, baths, etc.), keeps residents clean/dry, assists with transfers and mobility.
• Prepares residents for meals, serves trays, feeds as needed, ensures all residents served, wipes tables.
• Maintains tidy care units, changes beds, clears clutter, secures residents' personal property.
• Takes vital signs/weight/height, records intake/output, collects routine specimens, administers enemas.
• Assists with rehabilitative measures and transports residents as needed.
• Maintains bowel/bladder retraining programs and post-mortem care when required.
