Job summary

Heritage Woods of Rockford is seeking a Part-time CNA to provide compassionate, resident-centered care on the 1st shift. You will assist with Activities of Daily Living, following individualized care plans to promote independence and dignity.

You will report health changes to the clinical team, engage with residents and their families, and participate in ongoing education while maintaining strict confidentiality of resident information. Benefits and support are provided.

Qualifications

• High School graduate or GED equivalent required.
• Current certification from an accredited Certified Nursing Assistant (CNA) program.
• Exceptional communication and interpersonal skills.
• Must demonstrate the ability to serve with love, compassion, and dignity.

Responsibilities

• Deliver exceptional and individualized care as determined by each resident’s care plan.
• Provide assistance with Activities of Daily Living (ADLs) while promoting independence and maintaining dignity.
• Report changes in resident health status to the clinical team.
• Connect with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience.
• Engage in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experiences.
• Comply with all rules and regulations regarding confidentiality and privacy of resident information.
• Perform any other duties as assigned.
