Brookstone Estates, Emerald Glen, & Grand Victorian in Rockford, IL is seeking a Part Time Certified Nursing Assistant (CNA) to provide compassionate, resident-centered care based on individualized care plans. You will assist with ADLs, monitor health changes and communicate with residents and families in a caring manner.

Qualifications

• High School diploma or GED required.
• Current CNA certification from an accredited program.

Responsibilities

• Deliver exceptional and individualized care as determined by each resident’s care plan.
• Assist with Activities of Daily Living (ADLs) while promoting independence and dignity.
• Report changes in resident health status to the clinical team.
• Connect with residents and family with compassion to create a positive experience.
• Engage in ongoing learning through seminars, staff meetings and training.
• Comply with confidentiality and privacy of resident information.
• Perform other duties as assigned.
• Support a positive, respectful care environment.
