Job summary

Responsibilities

• Follow client’s individualized care plan as directed by supervisor.
• Assist with activities of daily living; housekeeping, meal prep, meds assistance as delegated.
• Provide personal care: bathing, grooming, toileting, mobility support.
• Transport and accompany clients to appointments and errands.
• Maintain accurate client records and report changes to care management.
• Ensure a safe, clean, and supportive home environment for clients.
