Qualifications

• Must have CNA certification from an accredited program.
• High School Diploma or GED equivalent.
• Excellent communication and interpersonal skills.
• Ability to work across departments and adapt duties as community needs shift throughout the day.

Responsibilities

• Deliver patient-centered care according to each resident's care plan.
• Assist with Activities of Daily Living (ADLs) while promoting independence and dignity.
• Support residents beyond direct care by helping with laundry and memory care activities.
• Report changes in resident health status to the clinical team.
• Connect with residents and family members in a compassionate, impactful manner.
• Engage in ongoing learning through seminars, staff meetings, and other training.
• Maintain confidentiality and privacy of resident information.
• Perform any other duties as assigned.
