Job summary

Reserve Care is seeking compassionate Home Care Aide to provide personal care under nurse supervision, following each patient’s plan of care. You will assist with activities of daily living, ensure safe environments, and support families in a home care setting.

The role requires an active STNA and strong English communication. Expect flexible shifts, weekend availability, and participation in ongoing education and on-call rotation as needed.

Qualifications

• Must be able to read and write English proficiently.
• Must be State Tested Nursing Assistant in good standing.

Responsibilities

• Provide personal care and assist with ADLs according to the plan of care.
• Maintain a safe patient environment and assist with transfers.
• Assist with ROM exercises and pre-poured medications under supervision.
• Record observations in the clinical record and complete charting and logs.
• Participate in care conferences, meetings, and education hours.
• Coordinate with the nurse and team to support patient and family needs.
