Job summary

Sanford Health in Iowa is seeking a Nursing Assistant (NA) to provide resident-centered care in a long-term care setting. The NA works under the supervision of an RN, performing basic health care tasks, daily living assistance, and support to residents, while adhering to care plans and infection prevention policies.

The role requires a CNA certification or enrollment, 18+ years old, and possible Colorado QMAP where applicable.

Qualifications

• GED or high school diploma preferred, but not required.
• Minimum of 18 years of age.
• Prior clinical or nursing assistant experience in long-term care preferred.
• CNA certification with the State Board of Nursing, or state certified CNA within four months of date into the position by enrolled training or certification process. CNA training and certification provided by facility.
• Colorado: Depending on facility, QMAP certification with the Colorado Department of Public Health & Environment required.
• Satisfactorily completes yearly department specific competency testing and maintains continuing education credits (12 hours annually).
• Where applicable, may be registered with the Unlicensed Assistive Person (UAP) Registry or Sanford Category II Driver requirements.

Responsibilities

• Assists residents with daily living activities and care plans.
• Records vital signs and documents interactions.
• Assists with transfers, repositioning, and mobility.
• Supports meals and hydration, ensuring a positive dining experience.
• Follows infection control policies and PPE use.
• Communicates changes to the charge nurse.
• May participate in committees or task forces as needed.
