Job summary

Qualifications

• Certified Nursing Assistant in good standing with the state (preferred, not required).
• High school diploma or equivalent.
• Current CPR/BLS certification
• Current Food Handler Permit (when applicable)
• No disciplinary action against certification
• 1 year of CNA experience in SNF/ALF preferred, not required.

Responsibilities

• Provides supportive services to nurse, med tech and other staff as needed and performs duties as assigned.
• Assists resident with activities of daily living per care plans and policies.
• Assists with lifting, turning, moving, positioning, transporting residents.
• Coordinates dining room services at meals, including setup, delivery, feeding, and documenting intake.
• Delivers nutritional supplements and documents intake.
• Tracks resident weight and reports changes to nurse/supervisor.
• Monitors skin condition and reports issues to nurse/supervisor.
• Assists with toileting programs and completes flow sheets daily.
• Maintains communication with nurse and supervisor about barriers to care.
